Enter Your Identifying Information:
1. Group Number - enter the Group Number as printed on your Dental ID Card.
Example: 987654  
2. Subscriber Id - enter the Subscriber Id, begin with the letter "W" in your subscriber ID, followed by 8 numbers.  Do Not enter any letters that may appear before the letter "W".  For example if your Subscriber ID is YPPW12345678, enter W12345678
3. Date of Birth - enter your Date of Birth. Please follow the format of MM/DD/YYYY.

Enter your Contact Information:
1. First Name, Middle Initial and Last Name - use the same information that appears on your ID card. Do not use nick names.
2.  Email address - enter your email address. If you do not have an email address enter: noemail@none.com

Select a unique user name and password:
1. User Name - Create a unique User Name,
Recommendation: at least 6 characters.
2. Password - Enter the Password twice.
Recommendation: at least 6 characters, use Numbers and Letters. Password are Case Sensitive.  For example, the letter "n" would not match the letter "N" in the Password: 123#N#456 .
- Please make a record of this User Name and Password.  If lost, you will need to call the Customer Service Number on your ID Card.  The Representative will reset your status and you may then re-register.

Dental Blue Select Members

Enter Your Identifying Information:
1. Group Number - enter the Group Number and your Division Number as printed on your Dental ID Card.  For example, if your Group Number is: T999999999 and your Division Number is 0001 then you would enter T999999999-0001 in the Group Number field on the Registration Page.
2. Subscriber ID - enter the Subscriber Id as it is printed on your Dental ID card.  Your Id will consist of the letter "M" followed by 9 numbers.  For example: M123456789. You will enter: M123456789 as your Subscriber ID.   
3. Date of Birth - enter your Date of Birth. Please follow the format of MM/DD/YYYY.
